KidsInclusive – Kingston Health Sciences Centre at Hotel Dieu Hospital (HDH) is committed to protecting your child’s personal health information. Traditionally we communicate with families by regular mail, fax and phone. While these are still our preferred methods of communication from time to time communication via email is requested. KidsInclusive has strict rules about what can be shared via email.

Please review the following information carefully before providing consent for KidsInclusive to use email as a form of communication with you. If you have any questions please do not hesitate to ask your service provider.

I understand that e-mail messages are not encrypted on the KidsInclusive/KHSC email system and security and privacy can never be completely guaranteed. For this reason, I understand not to use e-mail to communicate certain sensitive types of information, such as personal health information. I may also have other types of information that I would prefer not to be discussed in e-mail messages and I will consider other ways to communicate more sensitive and confidential information with KidsInclusive.

I understand that the use of email to share client files or reports without proper safeguards is strictly prohibited.

I understand that KidsInclusive will only use ‘initials’ to identify clients in any email communication.

I understand that the Information and Privacy Commissioner of Ontario does not support the practice of communicating personal health information via e-mail.

I understand that e-mail communication/contents may be retained as part of the electronic client record and becomes the property of KidsInclusive.

I understand that in saying yes to e-mail communications, I am providing KidsInclusive - Kingston Health Sciences Centre permission and my full consent to communicate with me via e-mail. I understand that I can withhold or place conditions upon my consent without concern of reprisal. I understand that this consent remains effective unless revoked by me in writing, or until my child is discharged from the KidsInclusive. I understand that the withdrawal of consent shall not have retroactive effect.
