Getting Started
Welcome to the Niagara Children's Centre Birth to “School Start” Core Services (Occupational Therapy, Physiotherapy, Speech-Language Pathology) Referral Checklist. This is an online checklist that will help you to determine whether a child who has not yet started school in the Niagara region may be eligible for Occupational Therapy, Physical Therapy, and/or Speech-Language Pathology assessments, at the Niagara Children's Centre. This takes about 10-20 minutes to complete.
